The Brandy Lift was developed as a means of lifting hair from the back and sides of the scalp onto the top of the scalp.  The procedure resulted in a slot of bald surface area that required rotational flaps to conceal.
Acell is a FDA approved bi-modal extracellular matrix useful in regeneration of tissue.  For this reason I began working with this product in FUE extraction sites.  The objective of this presentation is to stimulate interest in Acell by other physicians with the hope that we will eventually generate clinically significant data to prove or disprove the early clinical findings as they relate to Acell.
This study suggests that we should further study the benefits of Acell in body hair transplantation to see if we can further improve the survival rate of the body hair grafts.
A recent report on the use of ACell with plucked hairs suggests that plucking hair can result in follicle cloning.  This is called “autocloning” by the author.  I’m issuing this response because I am beginning to get phone calls requesting treatment with plucked hairs that are tainted with ACell based on presentations by Dr. Jerry Cooley and Dr. Gary Hitzig along with irrational exuberance on the part of some physicians who incorrectly feel these presentations represent some sort of clinical breakthrough in the treatment of hair loss.
Everyone has seen the amazing transformations, from a nearly bald head to one with thick and natural hair.  Unfortunately, we’ve all also seen more sobering ‘after’ photos depicting men who have received grizzly-looking results following poorly-performed procedures using crude techniques.  Either way, it is hard to deny that we are fascinated by hair transplant before and after photos.

The size of punches is an important subject in follicular unit extraction and follicular isolation techniques. It is also a strong marketing tool for some groups who attempt to mislead patients into believing that they can always use a small punch and they always get a high yield with little destruction to the follicles and follicular units. There are three different types of three hair natural follicular groups. When performing FUE we often run into individuals who do not desire additional strip scars or the risk of a wider scar. Sometimes they have a limited donor supply. This may risk exposure of the donor scars.

Doing single blade harvesting you can better control the blade angle during the scalp incision and make it parallel to the hair shaft, thus avoiding hair trans-section.
The basic unit of hair restoration surgery is the pilosebaceous unit. Dr. Jung Chul Kim called this pilosebaceous unit a "bundle graft" long before it acquired its present name, the follicular unit, by hair restoration surgeons. The objective of graft dissection is to remove intact follicular units from an excised strip or strips of donor tissue in a manner that insures the highest probability of survival. One may go further to state that the objective should assist in the overall cosmesis and natural appearance of the hair transplant.
Hair loss is caused by a gradual miniaturization of the hair follicles under these influence of multiple complex biochemical pathways in genetically sensitive individuals.  We know that males must possess the hormone Dihydrotestosterone (DHT) to have hair loss.
Hair loss due to disease, scarring and in particular, androgenetic alopecia, has troubled members of the human race since the dawn of history. A prescription for restoring hair is included in the Ebers papyrus of ancient Egypt which has been dated at 1800 B.C.
To achieve this result, I used a narrow band filter of that absorbs all light waves above and below 470 nanometers. This means it transmits only light waves in the narrow range (plus or minus 10 nanometers) above and below 470 nanometers. The absorbed frequencies of light are converted to heat. The incident light was from a fiber optic source that emitted white light. When the white light passed through the narrow band filter of 470 nanometers, the transmitted light was in the cyan range on the electromagnetic spectrum.

This is an acceptable means of cutting grafts, but we are attempting to take the technology one step further. Since a yellow structure absorbs blue light waves, it reflects red and green light. If a yellow structure has an incident light that is cyan (blue and green), it will absorb the blue and appear green. The colorless hairs will absorb no light waves and appear cyan. Thus, the proper incident light can create contrast between white hair and the surrounding structures.
Fortunately, in our offices we always try to improve our technology and created a better result for our patients. Therefore, I created a fiberoptic backlighting source of light waves that are transmitted through the tissue. In addition the light source is at an angle to the tissue so a slight degree of refraction of light occurs also. The transmitted light results in a slight shadow in the center of the hair, where the density is greatest and light transmission is reduced.
The color of a pair of pants depends on the light waves it absorbs. For instance a pair of blue jeans is blue because it absorbs all the wavelengths except blue. The color blue is reflected and the jeans appear blue.
This leads to the principals of color addition. Color addition involves combining different colors or wave lengths of light to form a new color of light. For instance, a combination of Red and Blue light result in the color Magenta. Red and Green forms the color Yellow and Blue and Green light result in the color Cyan.
The primary colors of light are defined as any three colors or frequencies of light that produce white light when combined in the correct intensity. The primary colors of light are Red, Green, and Blue. When these wavelengths are combined properly they produce white light.
The speed of light is a vacuum is defined as "c". The speed of light in a more dense medium is always less than c. As light passes through a medium its energy results in a non-resonant vibration prior to emitting the light. Some of the light energy is typically lost from the light rays and absorbed by the denser medium.
In this paper, I will rely heavily on the use of rays to represent the direction which a wave is moving. While we know that light is a wave (and not a stream of particles), I still use a line segment with and an arrowhead (i.e., a ray) to depict the refraction of light. The ray is constructed in a direction perpendicular to the wavefronts of the light wave; this accurately depicts the light wave's direction. The idea that a light wave can be represented by a ray is known as the ray model of light.
Rather than disappearing (and thus violating energy conservation), the energy carried to the boundary is divided up into a reflected pulse (which remains in the less dense medium) and a transmitted pulse (which passes across the boundary into the new medium). The reflected pulse has several noteworthy characteristics.
Violet light and the color violet is at the lower end of the visible spectrum. Green and Blue are in the middle of visible spectrum of light. Red is at the upper end of the spectrum. This means that violet light has a shorter wave length and higher frequency and that red light has a longer wave length and short frequency.
For each type of reflection, each individual ray follows the law of reflection. However, the roughness of the material means that each individual ray meets a surface which has a different orientation. The normal line at the point of incidence is different for different rays. Subsequently, when the individual rays reflect according to the law of reflection, they scatter in different directions. The result is that the rays of light are incident upon the surface in a concentrated bundle and are diffused upon reflection.
Most physicians use light which originates from above the tissue. This is termed top lighting. The incident ray reflects off the tissue and enters the retina of the graft cutter. If the hair has pigment and if the tissue is both thin and translucent, the graft cutter may produce very good grafts. If the graft cutter uses a microscope the proficiency of graft cutting my improve even better. Unfortunately, the majority of graft cutters do not use microscopes or even adequate top lighting. Therefore, the risk to the grafts increases in such cases. There is no excuse for this behavior in today's surgical procedure.
It is the law of reflection that governs most graft dissection and is applied by the majority of graft cutting technicians. It is also the least effect means of cutting grafts and produces the greatest risk to individual hairs. Since we all know that the total number of movable hairs is finite, this is of paramount importance to hair restoration surgery. If the tissue is cut incorrectly, hairs can be irreparably damaged and destroyed.
In an effort to improve the visualization of white hairs in hair restoration surgery, we began experimenting in the field of optics for methods to enhance the appearance of white hairs. This led to the formation of our Glow Chamber. This new invention has commercial applicability, but requires a far more intense light source than we have been able to create thus far.
Each individual wavelength within the spectrum of visible light wavelengths is representative of a particular color. That is, when light of that particular wavelength strikes the retina of our eye, we perceive that specific color sensation. Isaac Newton showed that light shining through a prism will be separated into its different wavelengths and will thus show the various colors that visible light is comprised of. The separation of visible light into its different colors is known as dispersion.
Electromagnetic waves are waves which are capable of traveling through a vacuum. Unlike mechanical waves which require a medium in order to transport their energy, electromagnetic waves are capable of transporting energy through the vacuum of outer space.

We do everything possible to meet these expectations and stand behind our work. We have not heard from this patient and we have not seen him back in our office. I will say that it is very hard to make any recommendations or give any advice to our patients in a chat room forum.
Hair restoration surgery in the young patient can be dangerous. We do not have a crystal ball and we cannot predict your future. If you begin surgery when you are young, you may find your hair loss exceeds your donor capacity. At this point you may wish that you had not begun doing surgery and simply desire to return to your natural state of baldness. Unfortunately, I find many individuals present to my office with this very complaint. They simply want to return to a natural, bald state.
Androgenetic alopecia (AGA), also known in women as female pattern hair loss, is caused by androgens in genetically susceptible women and men. The thinning begins between ages 12 and 40 years, the inheritance pattern is polygenic, and the incidence is the same as in men.
Both dutasteride and finasteride arrest the disease process of men with BPH and an enlarged prostate and are indicated to improve symptoms, reduce the risk of AUR, and reduce the need for BPH-related surgery.
The pharmacokinetics of dutasteide have been extensively studied in helthy volunteeres as well as in patients with BPH.

There were no serious adverse events that were considered drug-related.

As you may recall from your other reading on our site, growing hairs are in a constant cycle. An anagen hair (active phase) becomes a catagen (intermediate phase), which then becomes a telogen (dormant phase), then back to anagen again.
This individual has a double whorl. The dominant whorl is on the right side and is counter-clockwise. I call it dominant because it affects the majority of the crown and it overwhelms the left side whorl. The left side whorl is clockwise.
One of the most common complications observed after hair transplant surgery is forehead edema (swelling) and subsequent periorbital ecchymosis (bruising around the eyes). This occurs as fluid from the recipient area migrates downward along tissue planes from the top of the head to the upper forehead, then to the area above the eyebrows and eventually down around the eyes, where the appearance of bruising can be cosmetically distressing to the patient.
Density studies without regular patterns create areas of high densities and areas of low densities. This was confirmed in my own patients where I average 45 grafts per square centimeter.
Perhaps the most significant advancement in hair restoration surgery this year has been in the area of corrective procedures for prior pluggy looking hair transplants. We have successfully removed grafts using Follicular Isolation in a much more rapid fashion than we could remove them in the past.
