The area on the top of the scalp initially had hair growing in it from the strip procedure, but later became completely bald. The loss of hair in this area was correlated with the application of topical combination of Minoxidil and Retin A. The patient noted that the area gradually became irritated and red with the development of a crust. Scarring alopecias can produce skin changes and hair loss similar to this, but one often is not able to get additional grafts to grow in the area of scared skin. These combinations of Minoxidil and Retin A can be quite irritating to the skin. There is no strong evidence that Retin A adds any additional benefit to the Minoxidil. This patient later discontinued using the combinantion of Minoxidil with Retin A and later peeled the scab off. The area was left without hair and the skin had the appearance of scar tissue. Another contributing factor for this patient’s need of CIT hair transplant method was the lower donor density due to strip surgery from the past. Beginning in 2005, the IHTI surgical staff successfully transplanted 2000 CIT grafts to treat this patients’ hair loss. The treatment plan consisted of three smaller CIT sessions to achieve ideal coverage on the receded areas. This patient noted growth within just two months after his first procedure which in not uncommon as all hairs do not grow at the same rate. The patient’s previous procedure left the hairline very “pluggy” looking and noticeably mis-angled by a strip (FUT) clinic. For our surgical staff to provide corrective treatment on this patient, a plug redistribution session was necessary. In this repair session, we removed large follicular units (2 to 5 hair), dissected them and placed them back onto the hair line for a softer appearance. Often times patients want us to place these dissected follicular units back onto their donor areas to disguise scarring. Being very displeased with his hair line, he contacted us to arrange repair. The purpose of this session was to remove the large grafts on the hair line. The session resulted in the redistribution of approximately 170 plugs that yielded approx. 371 grafts for placement back onto his frontal scalp via CIT. At 12 months post-op, you can now see a more natural result after one day of CIT repair. We are excited to present another successfully treated repair case. Repair can be more difficult to accomplish than a usual hair transplant session. Repair cases also may require more than one session to reach each patient’s goals. The patient is a class 3v with straight, medium coarse, salt and pepper hair. Before coming to our clinic, the patient had 2 strip sessions and a scalp reduction performed during the mid 90’s. The patient’s goal was to soften the frontal hair line and add coverage to the frontal third. Dr. Cole successfully treated this patient with 1000 CIT grafts to the frontal scalp. The video / photo results shown below depict this patient’s progress at 10 months post-op. A dramatic change in the appearance of the frontal scalp has been achieved by 1000 CIT grafts. Also attached is video footage of the patient’s donor area previously harvested by approx. 1800 strip (FUT) grafts and 1000 CIT grafts. CIT scarring may or may not be visible in comparison to the strip scars that are visible in this patient’s donor. This is an advantage of beginning and ending hair restoration with CIT hair transplant method. With CIT, there is no linear scarring associated with strip hair transplant method (FUT). No clinic can guarantee that a surgery is 100% scar free but there are less invasive methods to harvest a donor area. A hair transplant should be performed by hair transplant methods that produce the best aesthetic result in both the donor and recipient area. Through the years, the patient’s hair loss progressed and the loss of native hairs on the scalp were replaced with newly transplanted hairs to add density in the receded areas on top of the scalp. Last year, the patient came to Dr. Cole for about 1500 transplanted CIT grafts to maximize the patient’s previously strip harvested donor resources. In 2007, the patient’s goal was to add density to the top, frontal scalp and temporal points. This patient’s donor area is comprised of a very fine caliper of hair with slightly above average hair density of about 190 hairs per cm2 with 180 being average. As you can see, the recipient area requires larger and smaller follicular units of better quality for optimal coverage. The patient recently came in for a touch-up to the frontal hair line and is very pleased with the outcome of the CIT procedure. The patient reported that the CIT was much less painful, caused less soreness and was much less invasive than his prior procedure. This patient looks much more youthful at 12 months after the CIT treatment and is expecting to see thicker hair in the coming months. At age 35, he wanted to fill in the recessions, redefine the frontal hairline, and give some light coverage to the thinning crown. He suffered progressive hair loss during this period of time, which, in retrospect, would probably have him at about a class V had he not undergone HT surgery. It is a good teaching case for demonstrating different philosophies about HT surgery. Between early 2004 and late 2005, he had three smaller procedures, which together accounted for 4800 grafts, all via FIT technique. Each time he was seen, we noticed progression of the hair loss, mostly manifesting as loss behind the frontal forelock (the "C"-shaped pattern so common with class III's as the recessions move back and centrally), and also widening of the crown loss. Eventually, the middle third on top was beginning to go, too. This gentleman is a good example of a pattern which is not uncommon. The hair loss is not aggressive early on (teens and twenties) and may not manifest at all until later in life (30's or even 40's), but may progress relatively rapidly to a class V. I know, it happened to me.
