Dr. Cole is coming to Manhattan, NY on Saturday June 30th. If you have never had a procedure with Dr. Cole then please either email or call to be scheduled for a consult and the appointment. If you are already post-op patients of Dr. Cole's then call or email for a late in the afternoon appointment with Dr. Cole.

Those of you that have not filled out the Patient Information Medical History form that is attached to this email and returned to us need to do so prior to scheduling the consultation appt. You may email or fax it back to 678-566-1957.
