Multipule Consultations In the US From March to May 2008

Dr. Cole will conduct 6 consultations In New York, Washington, Orlando and Los Angeles starting from march 2008, here are the dates:

March 15th – New york, NY
March 29th – Los Angeles, CA
April 5th - Orlando, FL
April 19th – Washington, DC
April 26th. – New york, NY
May 10th – Washington, DC
May 17th– New york, NY

If you have never had a procedure with Dr. Cole then please either email or call to be scheduled for a consult and the appointment. If you are already post-op patients of Dr. Cole’s then call or email for a late in the afternoon appointment with Dr. Cole.

Those of you that have not filled out the Patient Information Medical History form that is attached to this email and returned to us need to do so prior to scheduling the consultation appt. You may email or fax it back to 678-566-1957.
